What is Hospice?
Hospice cares for the person, not the disease. Hospice reaffirms life. Hospice focused on dignity. Hospice brings comfort and quality to the remaining months, weeks and days. Hospice does not hurry death. Hospice does not prevent death.
Hospice is a team of health professionals caring for the patient, and relieving anxiety for the caregiver/family. Our hospice support team helps patients and families with pain and symptom management, personal care and social, emotional and grief issues.
Hospice offers guidance through the unknown. Hospice makes connections with needed resources. Hospice brings friendship and comfort throughout the remaining time. Hospice continues after the patient’s death by providing support to the caregiver. Hospice is not a place, but a philosophy of care based on a whole person concept of living and dying. It is a unique health care approach in which the patient and family are the focus, not the disease.
Who Provides Hospice?
Our hospice team is a group of highly skilled hospice professionals and community resources. Because hospice focuses on the patient and family, and not the disease, our hospice team creates an individualized care plan based on the specific needs, wishes and values of the patient and family.
This comprehensive plan of care is directed by the patient and carried out by our hospice team, which may include but is not limited to:
- Patient and Family
- Experienced Nurses
- Chaplain/Bereavement Counselor
- Patient’s Physician
- Social Worker
- Medical Director
- Hospice Aides
- Trained Volunteers
Through the assistance of the hospice care team. Patients and families are able to control what at first seems uncontrollable. With pain and symptom management, as well as education and support, patients and families are able to focus on what is truly important When Life Matters Most.
Who is eligible for Hospice?
Hospice care is for people who have a life expectancy of six months or less, and have chosen to focus on pain and symptom relief over healing treatment.
Where is Hospice Care Provided?
Because hospice is a plan of care, not a place, patients may receive hospice care wherever they call home:
- Home or Apartment
- Assisted Living Facility
- A relative or friend’s home
- Nursing Home

Services
Services which may be provided under the direction and orders of your physician include:
Physician/Nursing Care
- Advance knowledge of pain management and symptom control
- Registered Nurse case manager coordinates all care needs, provides education, and support
- Clear communication of care plan and patient needs with caregiver and family
- Spiritual Services
- Nursing staff on call 24 hours a day, 7 days a week
- Five day in-patient Respite Care for caregiver relief as deemed necessary; General In-patient and Continuous Care for symptom relief
Spiritual Care Coordinator/Chaplain
- Spiritual support
- Emotional and Spiritual Comfort: In addition to physical pain, patients may experience emotional and spiritual pain. Patient can experience many losses such as loss of control over one’s own life, loss of relationships due to unresolved conflicts, and loss of purpose for one’s life. One of our chaplains can assist patients and families in many ways.
- Liaison for family clergy & hospice
Social Work Services
- Helping family members cope with loss issues and caregiving demands
- Helping patients and families with legal and financial needs such as advance directive information; funeral planning; Medicare, Medicaid, and insurance form completion; and referrals to community agencies
Hospice Aide Services
- Assisting in personal care
- Works under the supervision of the Registered Nurse
Volunteers

Volunteers complete a thorough training program and can assist our patients and families in many ways to include:
- Provide support to the patient which could include companionship, shopping, and running errands
- Provide caregiver relief to allow time for personal needs
Bereavement Support
- Provides resources that help coping with grief before and after the loss of loved ones
- Provides up to 18 months of follow up services for hospice families
- Individual and Family counseling
- Support groups
Medications
- Medications necessary for pain and symptom management related to the terminal illness and related conditions are provided by the hospice.
Medical Equipment
- Durable medical equipment such as bed, oxygen, and assistive devices are provided to the patient based upon physical needs.
Supplies
- Medical supplies are provided to meet the patient’s care needs. These may include diapers, disposable bed pads, and gloves.
Criteria for Admission
To be eligible for hospice services:
- The patient has a terminal illness certified by their attending physician and the hospice Medical Director
- The patient is not seeking curative treatment and elects the hospice benefit
